Exam Request Form - IT Department

Our Exam Request Form is designed to collect information about your upcoming exams in order for us to provide you with a safe and convenient exam environment in the computer labs.

Exam requests are for faculty members using a name-based AUIS email, such as [email protected]. Student accounts cannot submit this form.

NOTE 1: Please submit your exam request at least 24 hours prior to your exam.

NOTE 2: To book and reserve classrooms, please contact Ms. Peru Hussein at [email protected] from the Registration Office

NOTE 3: When taking assessments that require students to use applications on the computer, you cannot use LockDown Browser with Moodle.

NOTE 4: With Moodle and LockDown browser, internet access cannot be blocked. The application already provides a secure way of taking the exam.

NOTE 5: All exams needs to be set up by instructors on Moodle.
